English Cocker Spaniel: Chronic Otitis, Musculoskeletal Pain, and Humeral Condylar Fissure

The English Cocker Spaniel is often described as a robustly healthy breed.
This reputation is not entirely false: the breed notably has a reduced risk of osteoarthritis and atopic dermatitis compared to the average.
However, veterinary literature also documents specific predispositions that are worth knowing about: recurrent otitis, significantly overrepresented musculoskeletal pain, and a specific elbow pathology.

Otitis Externa: The Breed's Signature Pathology
Prevalence Figures
Engdahl et al. ( Canine Medicine and Genetics, 2023 - PMID 37202773 ) analyzed data from 10,313 English Cocker Spaniels in primary care in the UK in 2016.
Otitis externa was the second most common pathology, with a prevalence of 10.09% (95% CI: 8.87-11.32).
Kaimio et al. ( BMC Veterinary Research, 2017 - PMC5330127 ) analyzed 98,736 dogs in 55 clinics in Finland: prevalence of 15.7% otitis in ECS, compared to 27.0% in American Cocker Spaniels and 31.8% in Welsh Springer Spaniels.
In this study, 9% of American Cocker Spaniels with otitis had to be euthanized, compared to less than 1% of ECS.
Engdahl et al. ( 2024 - PMC10795400 ) compared the predisposition profile of ECS to all other breeds.
Ear discharge had the highest odds ratio of all examined pathologies: OR 14.66 (95% CI: 7.73-30.80), far ahead of keratoconjunctivitis sicca (OR 7.64) and musculoskeletal pain (OR 7.06).
Mechanisms
Drooping, hairy ears reduce ventilation of the ear canal, promoting humidity and warmth.
The higher glandular density in the canal leads to significant cerumen production.
Note: the 2024 VetCompass study specific to ECS shows that the breed does not have a particular predisposition to atopic dermatitis.
The role of allergic diseases in triggering otitis in ECS therefore needs to be nuanced...
Otitis is frequently recurrent and bilateral.
It can progress to otitis media or require total ear canal ablation (TECA-BO).
Managing recurrent forms involves identifying primary factors (food allergy, atopy, foreign body, hypothyroidism), secondary factors (Malassezia, cocci, bacilli), and perpetuating factors (stenosis, tympanic rupture, glandular hyperplasia).
Important!Never treat otitis without a veterinary diagnosis.Some products containing aminoglycosides are ototoxic if there is tympanic perforation.This cannot be ruled out without an otoscopic examination!

Musculoskeletal pain: a strong but non-specific signal
Musculoskeletal pain is 7.06 times more likely in English Cockers than in other dogs (Engdahl et al., 2024). This is the third strongest predisposition in the breed.
This figure requires a nuanced interpretation.
The term is a generic diagnosis in veterinary records: it can cover osteoarthritis, ligamentous pain, muscular pain, or specific elbow pathologies such as humeral condylar fissure.
Important point: the same study shows an odds ratio of 0.37 for specific osteoarthritis, representing a 63% reduced risk compared to other breeds.
This apparent contradiction is probably explained by the fact that musculoskeletal pain is identified as a symptom before it progresses to radiographically confirmed osteoarthritis.
These interpretations remain hypothetical and not demonstrated by the cited studies.
Humeral Intracondylar Fissure (HIF): the specific pathology of spaniels
Humeral Intracondylar Fissure (HIF), formerly known as incomplete ossification of the humeral condyle (IOHC), is characterized by a sagittal fissure in the distal humeral condyle, separating the medial and lateral trochlear centers.
Spaniel predisposition and prevalence data
Davenport et al. (Veterinary Radiology & Ultrasound, 2023) analyzed humeral condylar fractures at their referral center and identified spaniels (ESS, ECS, ACS) and French Bulldogs as the two most represented groups.
The prevalence of condylar fractures is 0.62% for Cocker Spaniels and 2.25% for English Springer Spaniels.
Contralateral fissure was present in 58% of spaniels on CT scan of the elbow contralateral to the fracture, and intracondylar sclerosis was present in 95% of cases.
Moores (Veterinary Clinics of North America, 2021 - PMID 33446360) emphasizes that HIF can develop as a stress fracture in adult dogs whose ossification was previously complete.
A documented case shows the development of HIF 22 months after a CT scan showed a normal elbow.
A genetic component with a possible recessive mode of inheritance has been suggested in pioneering clinical series.
Clinical presentation and management
Three modes of presentation:
- forelimb lameness, often intermittent, improving after prolonged rest
- condylar fracture from minor trauma (sometimes a simple jump), the most feared form as it can occur without prior signs
- incidental discovery on imaging.
CT scan is the reference tool for diagnosis: standard radiography may miss early lesions.
The reference treatment is the placement of a transcondylar screw to stabilize the condyle and prevent fracture.
The prognosis is generally favorable, with implant complications documented in 2.5 to 10% of cases in the long term.

Note: there are currently no published epidemiological data on the exact prevalence of HIF in the general population of English Cockers. Available data comes from clinical series in specialized centers.

Any forelimb lameness in an English Cocker, even intermittent and even if it resolves spontaneously, warrants an orthopedic consultation. Without CT exploration, asymptomatic HIF can go unnoticed until a fracture occurs.
Other documented predispositions
Periodontal disease
First pathology observed in VetCompass UK data, with an annual prevalence of 20.97% (95% CI: 19.31-22.62%), more frequent in females.
All comprehensive dental care must be performed under general anesthesia (AAHA 2019 recommendations - PMID 30776257).
Obesity
9.88% of diagnoses in the ECS cohort (95% CI: 8.66-11.09%).
Overweight exacerbates joint stress, particularly relevant in a breed already at specific orthopedic risks.
Keratoconjunctivitis sicca
Second strongest predisposition in magnitude (OR 7.64 vs non-ECS).
Easily detectable and treatable if determined early: regular ophthalmic check-ups are recommended.
Behavior and coat color
The VetCompass study on ECS documented a significantly higher prevalence of aggression in males and solid-colored dogs than in bi-colored ones. This phenomenon, sometimes referred to as "Cocker rage syndrome," remains debated regarding its mechanisms, but the statistical association is documented. This is an epidemiological signal that does not predict an individual dog's behavior.

Global approach: anticipate rather than suffer
On the ears
Regular ear cleaning with a suitable veterinary product, without deeply inserted cotton swabs.
Thorough drying after each bath or water activity.
Immediate consultation at the slightest repeated head shaking or abnormal odor.
If episodes recur, a dermatological consultation can help identify an underlying allergy.
Never treat otitis without a veterinary diagnosis.
On the joints
Routine orthopedic assessment from 3 to 4 years old, including elbow palpation.
Any intermittent forelimb lameness, even fleeting and improving with rest, should suggest HII.
Regularly monitor body condition score.
On the eyes and teeth
Regular ophthalmological check-ups to screen for keratoconjunctivitis sicca. Daily tooth brushing and regular oral examinations under general anesthesia.
